• Description: Helps older adults and adults with disabilities preserve their independence, dignity, and choice. Provides information and referrals to beneficial services.Currently providing a limited iPad distribution. This program is for those with no mobile computer devices and/or no internet service. Aims to help one access: Telehealth and tele-mental health services Companionship Suicide prevention resources. Limited supply. Call to sign up for waitlist. Program ends December 31, 2023. Also offers, Fall Prevention Program. Provides resources such as: Handheld shower spray, toilet safety frame or risers, grab bars, shower seats and transfer benches. Also provides safety tips and instruction on how to make home safe.
